Johns Hopkins U.S. Family Health Plan
The US Family Health Plan (USFHP or the Plan) is a managed care program developed by the Department of Defense. As designated TRICARE Prime providers under a DoD contract, the Plan offers comprehensive health care benefits to members of the seven uniformed services, including active-duty family members, retirees and their family members, and survivors.
The Plan:
- Is a permanent TRICARE Prime option, sponsored by the DoD.
- Is available to active duty family members and retirees and retiree family members – you must be registered in the Defense Enrollment Eligibility Reporting System (DEERS) to receive military health system benefits.
- Delivers full TRICARE Prime benefits including medical, surgical and behavioral health services.
- Includes an extensive network of primary and specialty care providers, and local hospitals, including The Johns Hopkins Hospital.
- Includes prescription drugs exclusively through Rite Aid pharmacies and by mail order.
- Offers exclusive extras through designated provider networks:
- Two free dental cleanings per year and discounts on other dental services.
- Discounts on vision services and lenses and frames.
- Discounts on LASIK eye surgery and certain cosmetic surgery procedures from Johns Hopkins specialists.
- Enables you to choose:
- Your personal primary care physician at any of our locations
- Local participating specialists and hospitals

- Johns Hopkins specialists and medical facilities, including The Johns Hopkins Hospital.
- Is the most cost-effective TRICARE Plan:
- Annual enrollment fees for retirees and their family members are:
- $460 per year for a family
- $230 per year for an individual
- DoD Standard co-payments for physician visits, prescription drugs and other services.
